Death of a Spouse or Dependent

When coping with the loss of a loved one, there are certain benefit related changes you may need to consider and other items that may also need your attention at this time. The following information has been prepared to assist you.

  • Notify Human Resources of the death as soon as possible. We will provide you with the forms you need to file any life insurance claims, if applicable.
  • Discuss a leave of absence for the funeral and to attend to other matters related to your loss with your supervisor.
  • Contact the Health Advocate Employee Assistance Program (EAP) if you would like help handling the emotional issues resulting from your loss.
  • Review your health care Flexible Spending Account (FSA) and/or Dependent Care Flexible Spending Account to determine if a decrease in, or cessation of, contributions are appropriate.
  • Update your medical and other benefit plans to remove the dependent from coverage.
  • Review your beneficiary designations for your life and retirement plans.
  • Update W-4 information in Workday to adjust your federal and state income tax withholding status, if necessary.
  • Contact the Social Security Administration to determine if you are eligible for survivor benefits.
  • Call your dependent's attorney, accountant, or financial planner and obtain pertinent documents and financial details.
  • Check with the local Veteran's Administration office if your dependent had service in the Armed Forces regarding possible survivor benefits.
  • If you move or change your phone number, update your information in Workday.
  • Review and update your emergency contact information in Workday, if necessary.
