Dental Insurance
Comprehensive dental insurance is provided by United Concordia, with two Preferred Provider Organization (PPO) options for coverage and no waiting periods for services. The main difference between the low and high option are benefits for major services and orthodontia under the high option.
Both options also include a Smile for Health--Wellness benefit for you or any of your covered dependents that have any of the following conditions: diabetes, heart disease, cerebrovascular disease (stroke), rheumatoid arthritis, lupus, organ transplant, head and neck radiation therapy, or current pregnancy.
United Concordia provides an interactive online member site for maintenance and inquiry for eligibility data, participating provider search, claim status, ID card requests, and more. Or, get the United Concordia Dental app and use MyDentalBenefits to find all your coverage info in one place online. You’ll see a quick overview right when you log in. Then just click to get details on everything from covered services to claims.
