Care Management

One of the many benefits of the Bucknell Faculty & Staff Health Plan is our ability to improve the efficiency of care as well as the patient experience. Our health care partner, Geisinger Health Plan (GHP), offers services through a team of registered nurses recognized as case and health managers. These skilled nurses are available to provide confidential concierge services to all Bucknell members. Whether you are looking for guidance with a newly diagnosed condition or you have an unexpected health event or hospitalization, GHP can provide the resources you need. How often and when you meet with a case or health manager depends on your specific situation. The nurses are available by phone or in person, and appointments can be made at your convenience. A nurse will contact you to schedule a time to meet, or you can call us at 800-883-6355.

This service will give you peace of mind knowing your care is coordinated by trained professionals who are experts at navigating the health care system. It is designed to provide quality care when and where you need it and improve your access to good care, guiding you through the health care system. For individuals with chronic conditions or complex medical needs, this program provides enhanced services, including:

  • Physician-led care team: Your physician leads a team consisting of you, your family, and all the health care providers involved in your care. All of your care is coordinated, whether you’re seeing your primary care physician or a specialist, or if you’re in the hospital, nursing home, or your own home. Our staff will help you connect with community resources, such as Meals on Wheels and Area Agency on Aging, and help with light housekeeping, if appropriate.
  • Dedicated nurse: A case or health manager is dedicated to your care and will work with you and your provider to establish a plan of care.
  • Plan of care: The team prepares a “plan of care” just for you. You will learn how to manage your condition, what symptoms to look for and when to call for assistance, receiving the care you need from health care providers you know and trust.
  • 24/7 access: Special phone numbers for your doctor’s office and care manager are available 24 hours a day.
  • Same-day urgent care appointments: Appointments for urgent care on the same day you call will ease your worries and avoid unnecessary trips to the emergency room.
  • Assistance with medication questions: Your case or health manager will help you understand medications, how to take them and what alternatives are available.
