Proof of Relationship Verification Process

Newly added dependents require submission of Proof of Relationship documents in order for their enrollment to be approved. Until these documents are submitted, the dependent’s enrollment is not complete. These documents must be submitted within the enrollment window (i.e., 31 days from hire or a life event) in order for the dependent to be eligible for coverage. If the forms are not received within the applicable enrollment window, the dependent is ineligible for coverage until the next enrollment period (life event or open enrollment period). Typical documents provided as proof of the relationship are listed below. You will be asked to attach the proof of relationship documents upon completion of your enrollment by scanning and uploading the document.
