If you or your covered dependents have any of the conditions above, you can sign up for Smile for Health—Wellness after your plan’s effective date (or whenever the diagnosis is made throughout the plan year—no need to wait until the next plan year).
After registering, you will receive a confirmation email. Then you can talk to your dentist about what additional services you may need to keep your mouth healthy.
